Job Description:
Supports OB/GYN offices in North Port, Port Charlotte, Arcadia and Punta Gorda Supervises about 15-20 Medical Assistants Monday through Friday from 8AM to 5PM Provides follow-up contact with patients to ensure compliance with recommendations, including medication reconciliations. Participates in planning, implementing, and collecting data for quality improvement initiatives. Engages in regular huddles and communication with practices to meet patient care needs. Develops workflow and protocols for hospital follow-up and reviews discharge information for the electronic medical record. Monitors patient outcomes and integrates clinical quality data with payer requirements. Collaborates with physicians, office staff, and management to report data and perform analytics reflecting quality metrics. Assures care gaps are met through motivational interviewing, implementation of standard orders and protocols, and patient education. Monitors care measures according to standards and documents and reports appropriately. Runs reports to identify patients who have not had appointments for up to two years and activates processes to schedule them. Assesses patient population needs through pre and post-visit planning, including coordination of community resource needs. Evaluates reports to identify patients in need of preventive care and documents, monitors, and reports efforts. Monitors Medicare patients for Annual Wellness Visits and establishes processes to schedule annual appointments. Performs other duties as assigned.

Education:
  • Associate's of Nursing [Required]
  • Bachelor's of Nursing [Preferred]
Work Experience:
  • 1+ years of position-related experience [Required]
  • 2+ years of position-related experience [Preferred]
Licenses and Certifications:
  • Registered Nurse (RN) [Required]
  • Basic Life Support
  • CPR Cert (BLS) [Required]
