Job Description Established fast past and growing Non-Medical, Private Duty Home Health Agency seeking an R.N. to assume the position of Client Care Coordinator. Pay is negotiable. Monday

•Friday, 8:30am-5:00pm, and on-call.

Main responsibilities to include:

Coordinate non-medical care for clients, write service care plans Supervise and coach direct care employees Work toward continuous quality improvement of services being delivered Develop and foster a strong culture of trust, professionalism, and mutual respect Maintain strong regulatory and compliance initiatives, policies, & procedures Create and maintain exceptional relationships with care staff, clients, and referral sources, community partners Expertly handle client or caregiver related issues, complaints, incidents, and investigations Work collaboratively with internal stakeholders including Scheduling, HR, Recruiting, and other support roles Provide valuable input to ensure the best match for client/caregiver compatibility Complete appropriate documentation in a timely manner Stay current with changing regulations, technology improvements, and industry trends EDUCATION/EXPERIENCE-Registered Nurse

•Client Care Coordinator Registered Nurse required Joint Commission or other accreditation organization experience preferred Experience working with the Agency for Health Care Administration (AHCA) Previous, successful management experience in a private duty and home health care setting required Prior scheduling experience preferred Proficient skills in Microsoft Office and or Google Suite Must possess and demonstrate excellent organizational, communication, interpersonal and leadership skills as well as a positive and professional image CERTIFICATION, LICENSURE, & REGISTRATION-Registered Nurse

•Client Care Coordinator Must possess and maintain a Valid Driver's License and maintain adequate auto insurance Registered Nurse, actively licensed in Florida

LANGUAGE

SKILLS-Registered Nurse

•Client Care Coordinator Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
