Aveanna Healthcare is seeking a compassionate Private Duty Registered Nurse to provide one-on-one skilled care to pediatric and adult patients in their homes. In this role, you will deliver clinically focused nursing, including assessments, medication administration, ventilator and trach management, wound care, and patient/family education. You'll collaborate closely with an interdisciplinary team, follow individualized care plans, and document in electronic records. Responsibilities Provide direct skilled nursing care to pediatric and adult patients in their homes per physician orders and care plans Perform assessments, monitor patient conditions, and document all care accurately in electronic records Administer medications, treatments, and therapies while ensuring patient safety and comfort Educate patients and families on disease management, equipment use, and home care best practices Collaborate with physicians, therapists, and care team members to coordinate and update plans of care Operate and manage home-based medical equipment such as ventilators, feeding tubes, and tracheostomies Respond to changes in patient status and manage emergencies following clinical protocols Maintain infection control standards and comply with all regulatory and Aveanna Healthcare policies Participate in ongoing clinical training, competency checks, and professional development activities Communicate regularly with supervisors and care coordinators regarding patient progress and needs Required Skills Skilled nursing assessment and care planning IV therapy and medication administration Ventilator and tracheostomy management Enteral feeding and G-tube care Wound care and infection control Electronic health record (EHR) documentation Patient and family education Emergency response and crisis management Care coordination with interdisciplinary teams Time management in home health settings
