Key Responsibilities:

Perform in-home annual health risk assessments for Medicare Advantage plan members Review and evaluate prior and current medical history, chronic conditions, and medication usage Offer education regarding medical conditions and prescribed medications to improve patient outcomes Identify care gaps and provide recommendations for follow-up, supporting ongoing patient wellness No responsibility for treatment planning or prescribing medications

Qualifications:

Valid Nurse Practitioner License in the state of Florida Current CPR certification (BLS) Board Certification as a Nurse Practitioner through AANP or
