Registered Nurse (RN) - Home Care Assessments TLC HomeCare Inc. is currently seeking a dedicated and experienced Registered Nurse (RN) to perform in-home patient assessments.Responsibilities:
- Conduct initial and ongoing patient assessments in clients' homes
- Develop and review care plans based on patient needs
- Coordinate with caregivers and healthcare professionals
- Ensure accurate and timely documentation
Requirements:
- Active RN license
- Experience in home care or clinical assessment preferred
- Strong communication and organizational skills
- Reliable transportation and willingness to travel
