Role:
We're seeking a detail-oriented, experienced MDS Coordinator (RN) to ensure accurate, timely assessment documentation in compliance with state and federal guidelines. As the MDS Coordinator, you'll coordinate and oversee the completion of Minimum Data Set (MDS) assessments and care plans for residents, ensuring regulatory compliance and optimizing reimbursement.
Key Responsibilities:
Complete and submit MDS assessments in a timely and accurate manner Coordinate interdisciplinary team (IDT) meetings to develop individualized care plans Monitor case mix index and ensure proper documentation for reimbursement Collaborate with nursing staff, therapy, and other departments to ensure comprehensive resident assessments Maintain compliance with federal and state regulations, including RAI process and PPS guidelines Educate and train staff on MDS documentation and regulatory updates Conduct audits and monitor quality indicators (QMs/QIs) Participate in Quality Assurance & Performance Improvement (QAPI) initiatives
Requirements:
Current LPN license in the state of Florida (required); RN license in the state of Florida is preferred Minimum 3 years of MDS Coordinator experience in a long-term care or skilled nursing facility (preferred) Knowledge of RUGs, PDPM, and Medicare/Medicaid guidelines Strong attention to detail and organizational skills Experience with electronic health records (EHR) systems (e.g., PointClickCare or similar) Excellent communication and team coordination abilities
