Job Description

Overview We are seeking a dedicated and reliable LPN or RN to join our Home Health team. The ideal candidate will provide high-quality nursing care to our peds. patients in their homes, ensuring their comfort and well-being. This role requires a strong clinical background, excellent communication skills, and the ability to work independently while collaborating with a multidisciplinary team. A strong knowledge of trach care, G-tube care, is a plus.
RELIABLITY IS A MUST
Responsibilities Conduct comprehensive patient assessments (RN Only) to develop individualized care plans. Administer medications and treatments as prescribed. Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes. Provide education and support to patients and their families regarding health management and treatment options. Document all patient interactions, assessments, and care provided in compliance with regulatory standards. Collaborate with physicians, and other healthcare professionals to coordinate patient care. Maintain a safe and clean environment for patients, adhering to infection control protocols. Experience Previous experience in home health working with medical fragil children., G-tube and Trach knowledge is a plus, will train. Excellent interpersonal skills with the ability to build rapport with patients and families. Ability to work independently while effectively managing time and priorities in a home health setting.
