PM Qualifications:

Active, valid RN license (state of practice) Minimum 1 year of recent home health nursing experience, preferably field-based Proficiency with Home Care Home Base (HCHB) electronic documentation Expertise with OASIS assessment tool Reliable transportation, valid drivers license, and auto insurance

Key Responsibilities:

Complete comprehensive OASIS assessments and develop tailored care plans Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management in patients homes Closely monitor patient status, promptly communicating changes to the broader care team and physicians Educate patients and families on medication, disease management, treatment plans, and preventive health strategies Document all assessments and patient interactions accurately and timely within HCHB Collaborate with therapists, medical social workers, and home health aides, coordinating holistic patient care Participate in ongoing case conferences, staff meetings, and continuing education initiatives Comply with agency and regulatory standards for care and documentation
