This vital role offers the opportunity to provide compassionate, high-quality nursing care directly in patients' homes, supporting children with complex medical needs and their families. As an RN in pediatric home care, you will be a key advocate for patient well-being, ensuring safe, effective, and personalized care plans are executed seamlessly. Your expertise will empower families, promote health stability, and enhance quality of life for pediatric patients requiring ongoing medical support. Responsibilities Deliver comprehensive nursing assessments and develop individualized care plans tailored to each child's unique needs. Administer medications, treatments, and therapies following physician orders while monitoring for efficacy and adverse reactions. Educate families and caregivers on medical procedures, medication management, and emergency protocols to foster confident home care practices. Coordinate with multidisciplinary teams to ensure smooth discharge planning, transition of care, and ongoing case management. Conduct routine vital signs checks, physical assessments, and diagnostic evaluations to monitor patient progress and adjust care as needed. Maintain meticulous medical records in compliance with HIPAA regulations and document all patient interactions accurately. Engage in utilization management activities to optimize resource use and ensure compliance with Medicaid guidelines. Requirements Valid RN license in the state of Georgia or relevant licensing jurisdiction. Critical care experience or background working within Level I or Level II trauma centers highly preferred. ICU, emergency medicine, or hospital medicine experience demonstrating advanced patient assessment skills. Experience with pediatric populations, including PICU (Pediatric Intensive Care Unit) or inpatient pediatric settings. Knowledge of medical terminology, physiology, and acute care protocols essential for managing complex cases. Strong understanding of discharge planning, medical records management, and post-acute outpatient care processes. Excellent communication skills to effectively educate families and collaborate with healthcare teams.
