What they do
A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

401(k) with match Full Support Staff Responsibilities Monday Friday schedule, 8:30 AM 5:00 PM Float across multiple service areas based on patient and staffing needs Perform patient admissions, routine visits, and symptom management Deliver compassionate, high-quality hospice care in patients homes Assess patient conditions and update care plans accordingly Collaborate with physicians and interdisciplinary team members Educate patients and families on hospice services and symptom management Maintain accurate documentation and ensure timely communication Travel daily within the metro Atlanta area Requirements Clear and active RN license in the state of Georgia Minimum of 1 year of hospice experience required Excellent clinical and communication skills Valid driver s license, reliable transportation, and current auto insurance Current CPR certification Flexibility to cover varying territories day-to-day About Us Titan Placement Group is a permanent placement healthcare recruiting firm dedicated to connecting healthcare organizations with high-quality candidates.
