Deliver comprehensive care in the community as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members. This per diem opportunity offers flexibility, allowing you to create your own schedule with shifts available Monday through Sunday and a minimum commitment required on a weekly or bi-weekly basis. Assignments are within Oglethorpe County, with visits to patients in various locations such as Lexington, Crawford, and Maxeys, tailored to residents' needs after discharge or as part of annual health evaluations.

Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage members Review patient medical history, medical conditions, and current medications Educate patients on health conditions and medication management Identify gaps in care and recommend steps for resolution Maintain documentation following standardized protocols Please note: No treatment planning or prescribing of medications is required

Qualifications:
