A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Community Health Network has been placing experienced nurses and physical therapists in home care, hospice, and case management for over 30 years. We offer a stable, supportive, and growing organization that focuses on your needs and interests. Well established home care agency has a need for an experienced Home Health RN Case Manager to see patients in homes in a coverage territory to include Forsythe county to N. Hall. Monday-Friday, 40 hours/week No weekends or holidays! Great pay rate with IRS mileage reimbursement Full benefits package available Guaranteed hours 13 week contracts (chance to extend contrat/stay on perm) Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency. The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
