Monday-Friday | Day Shift | Rotating On-Call (if applicable) Position Summary MDC Healthcare is seeking a compassionate, organized, and highly motivated Registered Nurse (RN) to join our growing team. The Field Nurse is responsible for providing skilled nursing services to clients in their homes while ensuring compliance with all state, federal, Medicaid waiver, and company regulations. This role requires excellent clinical judgment, strong communication skills, and the ability to work independently while collaborating with an interdisciplinary team. The ideal candidate is passionate about delivering exceptional patient care, maintaining accurate documentation, and building positive relationships with members, caregivers, physicians, and case managers. Essential Duties & Responsibilities Complete comprehensive nursing assessments, admissions, reassessments, supervisory visits, and discharge visits. Develop, implement, and update individualized nursing care plans. Perform supervisory visits for caregivers in accordance with Medicaid waiver program requirements. Provide skilled nursing services while maintaining high-quality patient care. Complete all documentation accurately and within MDC Healthcare's required timeframes. Document all visits in Home Care BOSS and other required electronic systems. Communicate effectively with physicians, case managers, caregivers, family members, and management. Educate members and caregivers regarding disease management, medications, safety, infection prevention, and available community resources. Report significant changes in condition to the physician and Director of Nursing immediately. Follow all infection prevention and control standards. Maintain HIPAA compliance and protect all Protected Health Information (PHI). Participate in quality assurance and performance improvement activities. Attend all required nursing meetings and mandatory training sessions. Respond professionally and timely to work emails during scheduled business hours. Maintain a professional appearance while representing MDC Healthcare. Assist with additional nursing duties as assigned.
