Field-based / Community (Home Health / Hospice Setting) Position Overview We are seeking a compassionate and experienced Registered Nurse (RN) Case Manager to provide high-quality, patient-centered care in a hospice/home health setting. This role is responsible for planning, coordinating, and delivering care to patients and their families while collaborating with an interdisciplinary team to ensure comprehensive support. Key Responsibilities Manage patient cases and provide clinical direction to the interdisciplinary team, physicians, and family members Develop, implement, and document individualized patient care plans Deliver care in accordance with established infection control and safety guidelines Ensure continuity and quality of patient care with accurate and timely documentation Monitor assigned cases to ensure compliance with regulatory and payer requirements Educate patients and families on care plans, treatment, and available resources Supervise hospice aides and licensed practical nurses (LPNs) Participate in on-call rotation and provide support to patients and families as needed Promote organizational policies, standards, and patient care philosophy Maintain professional competency through ongoing development and training Perform additional nursing duties as assigned Minimum Qualifications Graduate of an accredited Diploma, Associate, or Bachelor's Degree in Nursing Active Registered Nurse (RN) license in the state of employment Current CPR certification Minimum of 2 years of clinical nursing experience (home health or hospice preferred) Strong clinical assessment, communication, and problem-solving skills Ability to work independently and manage multiple patient cases Valid driver's license, reliable transportation, and auto insurance Ability to lift, move, and assist patients as needed Benefits Paid Time Off (PTO), Sick Leave, and Holiday Pay Comprehensive Medical, Dental, and Vision Insurance 401(k) Retirement Plan Gas Allowance and Vehicle Maintenance Stipend Employee Assistance Program (EAP) Tuition Reimbursement (for eligible employees) Career Growth and Advancement Opportunities Ongoing Training and Professional Development
