5-10 Admissions per Week Join Our Growing Home Care Team! We are seeking an experienced and compassionate Registered Nurse (RN) to join our home care team in the Cleveland, Georgia area. This position is primarily responsible for conducting admissions and initial assessments for new home care clients in their homes. The ideal candidate enjoys building relationships, educating patients and families, and ensuring a smooth start to care while maintaining compliance with state and agency requirements. This role offers flexibility and is perfect for an RN looking for supplemental income or a part-time opportunity. Responsibilities Complete initial home care admissions and comprehensive nursing assessments Develop and coordinate individualized plans of care Evaluate patient needs and determine appropriate services Provide patient and family education regarding care plans and services Accurately complete all required documentation in a timely manner Communicate findings and recommendations with agency staff and physicians as needed Ensure compliance with all agency, state, and federal regulations Participate in occasional reassessments or supervisory visits as needed Qualifications Active Georgia Registered Nurse (RN) license Minimum 1 year of nursing experience required Home care, hospice, case management, or community health experience preferred Strong assessment and clinical documentation skills Excellent communication and organizational abilities Valid driver's license and reliable transportation Ability to travel throughout the service area What We Offer Flexible schedule Consistent referral volume Supportive clinical and administrative team Opportunity to make a meaningful impact in patients' lives Competitive per-visit compensation Schedule Part-Time / Per Diem Approximately 5-10 admissions per week Flexible scheduling available
