Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage members Travel to patient homes within Douglas County, including Douglasville, Villa Rica, and Austell Review and document prior and current medical history, conditions, and medication use Offer education to members regarding their health needs and conditions Recognize and communicate gaps in care, providing actionable recommendations

Qualifications:

Active Nurse Practitioner license in Georgia Current CPR certification (BLS) Board certification through AANP or

ANCC MSN
