Key Responsibilities:

Perform comprehensive home health risk assessments for Medicare Advantage members Review and evaluate patient medical history, current medical conditions, and medication usage Educate patients and families about health conditions, disease prevention, and medication management Identify existing gaps in care and offer actionable recommendations to improve outcomes Travel to homes in Crawford, Lexington, Maxeys, and the surrounding areas within Oglethorpe County No requirement to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Georgia Current CPR certification (BLS) Board certified through AANP or

ANCC MSN
