Flexible options with shifts available every day, Monday through Sunday Self-scheduling to accommodate your availability Minimum weekly or bi-weekly commitment required

Key Responsibilities:

Perform comprehensive annual risk assessments in the patients home environment Review and evaluate medical history, current medical conditions, and medication usage Educate patients on their diagnoses and prescribed medications Identify gaps in care and offer guidance to close those gaps Focus on assessmentno ongoing treatment planning or prescribing is required

Qualifications:

Active Nurse Practitioner license in Georgia Board Certification through AANP or ANCC Current CPR certification (BLS) MSN degree from an accredited nursing program At least six months of Nurse Practitioner experience preferred This position offers a per evaluation payment structure ($80$90 per completed assessment), along with a mileage stipend and compensation for administrative time.
