Responsibilities:

Conduct thorough home health risk assessments for Medicare Advantage plan members, with each assessment taking approximately 45 minutes Evaluate patients past and current medical histories, conditions, and medication usage Offer education related to medical conditions and medication adherence Identify gaps in care and provide actionable recommendations to enhance patient outcomes Visits may include annual health assessments or post-discharge follow-ups Travel within assigned areas in Oglethorpe County, with potential scheduling in Lexington, Crawford, Maxeys, and surrounding communities Documentation and administrative requirements are included, but there is no expectation for you to develop treatment plans or prescribe medications

Qualifications:
