Ellijay, GA Seasons Care Hospice is seeking a compassionate and experienced Registered Nurse (RN) Case Manager to serve patients and families throughout the Ellijay, Georgia area. Pay and Schedule $50

•52 per hour Full-time Monday-Friday 8:00 a.m

•5:00 p.m. On-call rotation every fifth week Position Overview The Hospice RN Case Manager plans, coordinates, and delivers patient-centered hospice care. This position includes assessing patients' needs, developing and updating individualized care plans, implementing appropriate interventions, and collaborating with families and the interdisciplinary team to promote comfort, dignity, and quality of life. Responsibilities Complete initial and ongoing patient assessments Develop and update individualized plans of care Provide skilled nursing services in patients' homes or care settings Manage symptoms and support pain-control measures Coordinate care with physicians and the interdisciplinary hospice team Educate patients, families, and caregivers about the plan of care Maintain accurate and timely clinical documentation Communicate changes in a patient's condition to the appropriate team members Participate in the on-call rotation every fifth week Travel to patients in the assigned service area Qualifications Current and active RN license Hospice or home-health experience preferred Strong clinical, organizational, and communication skills Compassionate and patient-focused approach to care Ability to work independently and as part of an interdisciplinary team Reliable transportation and the ability to travel throughout the assigned service area Join a team committed to providing compassionate, high-quality care to patients and their families.
