Responsibilities Educate patients and families on dialysis modalities, kidney disease, and treatment options in a clear, compassionate manner Conduct one-on-one and small group education sessions in clinics, hospitals, and community settings Collaborate with nephrologists, nurses, social workers, and dietitians to develop individualized education plans Assess patient learning needs, readiness, and health literacy to tailor teaching methods and materials Document education encounters, patient understanding, and preferences in the electronic medical record Support patients in shared decision-making about in-center, home hemodialysis, and peritoneal dialysis options Coordinate follow-up education, referrals, and transitions between care settings Maintain current knowledge of dialysis therapies, guidelines, and patient education best practices Ensure compliance with regulatory, safety, and privacy standards (CMS, HIPAA) Participate in quality improvement, outreach, and staff training related to kidney care education Required Skills Patient education on kidney disease and dialysis modalities Clinical knowledge of chronic kidney disease and ESRDUnderstanding of in-center and home hemodialysis and peritoneal dialysis Care coordination and interdisciplinary teamwork Health literacy assessment and teach-back methods Motivational interviewing and counseling skills Electronic medical record (EMR) documentation Knowledge of CMS and ESRD regulatory requirements Presentation and group facilitation skills Cultural competency and communication with diverse populations
