About McIntosh Trail At McIntosh Trail Community Service Board, we offer individuals experiencing symptoms associated with mental illness, addictive disease and/or developmental disability the hope for optimal functioning and recovery by providing quality behavioral health services and supports. Summary Under general supervision, assesses client health problems and needs. Develops and implements nursing care plans and maintains medical records. Administers nursing care to ill, injured, convalescent, or disabled clients. May advise clients on health maintenance and disease prevention or provide case management. Responsibilities Perform walk-in assessments when individuals are present to the crisis service center. Notify appropriate staff when individual is admitted to the crisis stabilization unit/temporary observation unit. Complete required documentation for services rendered by utilizing the recognizable and acceptable documentation formats for nursing assessments, treatment plans and BHA's. Ensure documentation reflects the individual's treatment goals and objectives. Document in a clear and concise way in compliance with FY24 Relay needed information to psychiatric staff. Report sudden/new changes in individual's behavior to appropriate psychiatric staff. Complete/update nurse shift report for continuity of care. Ensure that individuals/visitors/staff are safe and away from dangerous or harmful material. Educate staff of the risk for contraband during the admission search and during visitation. Educate staff to report contraband found to the charge nurse/nurse manager. Ensure that equipment (bed, blood pressure machine, thermometers, accu-chek machine, etc.) is clean and in working order. Ensure that the Pyxis machine is working properly and report any problems to the pharmacist. Notify program assistant when OTC medication/supplies are getting low or when supplies are needed. Supervise subordinate staff to ensure required supervision and care of individuals is maintained throughout shift. Report issues that occur during the shift to the Nurse Manager or Director. Minimum Qualifications Bachelor's degree in nursing from an accredited college or university OR Associate degree in nursing from an accredited college or university AND current Georgia license as a Registered Professional Nurse AND any related professional work experience. Physical Demands may include Ability to lift and carry up to 25/50 lbs. Sitting, standing and/or walking, pushing/pulling, handling, reaching, bending, twisting, crawling, kneeling, squatting, climbing (ladders or stairs), overhead reaching.
