Conduct client assessment (History, Physical, Vital Signs). Monitor and report changes in client status or conditions as indicated by signs and symptoms. Participate in all phases of the nursing process including assessment, planning, implementation and evaluation for each client. Establish a working relationship with the client to assess his/her needs and plan a course of action. Assist client to and from therapy on time. Medication management and assistance Nurse will work with 1 client with a CNA as well. Some clients will be on the ventilator. Experience with trachs, vents, or ICU preferred.
