A Home Health Registered Nurse cares for patients in their homes or in community settings, primarily older patients and people with chronic illnesses and limited mobility. Works as part of a community or public health program to coordinate care for patients in between clinical appointments or treatments.
Looking for an RNs ONLY for a case in Hampton. 8 Hours a week (open to hearing nurse's proposed schedule).
Diagnosis of child:
Complex medical history including hypoxic-ischemic encephalopathy (HIE), developmental delay, chronic respiratory failure with tracheostomy dependence, seizure disorder, G-tube dependence for nutrition and medications, reactive airway disease, obstructive sleep apnea, muscle disease, trismus, cataracts, recurrent vomiting, and ongoing feeding difficulties requiring continuous skilled nursing care and monitoring. Pay $60 per
Hour Responsibilities:
Administer medications, monitor vital signs, assess patient responses to treatments and interventions, and promptly recognize and report changes in the patient's condition. Provide skilled nursing care for medically fragile pediatric patients, including the management and monitoring of tracheostomies (trach care), mechanical ventilators, suctioning, airway management, oxygen therapy, pulse oximetry, and enteral feeding as prescribed. Collaborate with physicians, respiratory therapists, therapists, case managers, and other healthcare professionals to develop, implement, and update individualized plans of care to ensure optimal patient outcomes. Maintain accurate, timely, and HIPAA-compliant medical records, care notes, medication administration records (MARs), and clinical documentation in accordance with agency, state, and federal regulations.
Requirements:
Valid RN license in GA. Familiarity with caring for children and young adults with special needs, including developmental disabilities and complex medical conditions. Strong communication and interpersonal skills to effectively interact with patients, families, and multidisciplinary teams. Ability to remain calm and composed in high-stress situations and handle emergency situations with confidence.
