Key Responsibilities:

Perform comprehensive annual home health risk assessments for Medicare Advantage members in various locations across Douglas County Assess clients prior and current medical history, medical conditions, and medication use Provide education tailored to each patient's medical conditions and medication regimen Identify gaps in care and offer clear recommendations for improvement Focus exclusively on assessment; there is no requirement to create treatment plans or prescribe medications

Qualifications:
