• As an RN at Epiphany Healthcare, you will play a vital role in delivering exceptional care to our patients.

You will collaborate with other healthcare professionals to develop and implement individualized care plans.

Your responsibilities will include:

• Key Responsibilities:
• Provide direct patient care, including administering medications, wound care, and monitoring vital signs.
• Assess and chart observations of the patient's condition at each visit.
• Complete evaluation tasks, including reviewing medication and vital signs.
• Educate patients and their families about health management and disease prevention.
• Maintain accurate and up-to-date patient records and documentation.
• Communicate effectively with patients, families, and healthcare team members.
• Participate in care plan development and implementation.
• Ensure compliance with all healthcare regulations and company policies.
• Qualifications:
• Current RN license in the state of Georgia.
• Previous experience in home healthcare or a similar setting preferred.
• Strong clinical skills and knowledge of nursing practices.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Experience Valid RN license with current state registration; additional certifications such as BLS (Basic Life Support) Demonstrated expertise in specialized nursing areas such as critical care medicine, cardiology procedures including catheterization or infectious disease care. Background working with individuals with disabilities or developmental delays; experience in memory care or dementia/Alzheimer's care highly valued. Familiarity with ICD coding (ICD-9/ICD-10), CPT coding for procedures, laboratory processing techniques, and infection control protocols. Experience delivering nursing care in outpatient clinics, urgent care centers, long-term care facilities like nursing homes or assisted living communities is advantageous. Knowledge of OSHA standards related to aseptic technique and sterile processing; ability to perform venipuncture and IV insertions confidently.
