Responsibilities:

Perform comprehensive home health risk assessments for Medicare Advantage members Assess medical history, current health conditions, and medication regimens Deliver tailored patient education on medical conditions and medication management Identify gaps in care, providing recommendations for improved health outcomes No requirements for treatment planning or medication prescribing

Required Qualifications:

Active Nurse Practitioner license in the state of Georgia Current BLS CPR certification Board Certification through AANP or ANCC Masters degree in Nursing from an accredited institution (MSN) Six or more months of current Nurse Practitioner experience is preferred
