Overview Registered Nurse
  • Home Health in Kaneohe, Hawaii.
This travel, temporary assignment starts 07/27/2026 for 13 weeks with a guaranteed 40 hours per week. Weekly pay ranges from $2,269 to $2,363. Shift pattern is 5 days per week, 8-hour shifts. The role operates in a home healthcare setting, focusing on patient-centered care and case-management activities for a diverse patient population.

On-call:
6 shifts per month as needed (including weekends) Weekend rotation: Rotating weekends as needed (2-3 per month) Responsibilities Provide excellent patient care within the home health setting and participate in the development, implementation, revision, and reporting of the case management program. Act as a liaison among patient, family, nurse, physicians, multidisciplinary team, and payer to optimize outcomes; serve as a consultant to the healthcare team on specific patient items. Lead the coordination of patient care across disciplines, monitoring the appropriateness and timeliness of services and ensuring the interdisciplinary care plan aligns with the patient's clinical course. Discuss with physicians the appropriateness of resource utilization, treatment plan, estimated length of stay, and discharge planning. Focus on complex patients, frequent ED utilizers, chronic pain and substance use patients, and homeless patients; collaborate with acute care case managers to ensure ongoing care appropriateness. Coordinate transfers to tertiary centers when needed, including transfer of patient information required for continuity of care. Provide oversight and collect data required for regulatory and accreditation compliance; conduct focus studies on frequent ED visits to inform care improvements.
