Contract Duration:
13 weeks
Guaranteed Hours:
40 hrs/week Responsibilities Conduct comprehensive patient assessments and develop individualized home healthcare plans. Administer medications, IV therapy, wound care, injections, and other skilled nursing treatments as prescribed. Monitor patient progress, evaluate changes in condition, and communicate updates to physicians and the interdisciplinary care team. Educate patients and caregivers on disease management, medications, treatments, and preventive care. Perform comprehensive OASIS assessments and maintain accurate documentation in the EMR. Coordinate care with physicians, therapists, social workers, and other healthcare professionals to ensure continuity of care. Assess home environments for safety risks and recommend appropriate interventions or medical equipment. Requirements 1+ year Medicare-certified home health agency experience
