Key Responsibilities Align care with Elara Caring mission and leadership direction Maintain HIPAA confidentiality standards Assess patient and family needs (physical, psychosocial, environmental) Manage full caseload including planning, coordinating, and evaluating care plans Perform initial and ongoing patient evaluations and update care plans Communicate changes in patient condition to clinical leadership and care team Revise and coordinate care plans with interdisciplinary team Requirements Current, unrestricted RN license 1+ year clinical nursing experience OASIS experience preferred Willingness to travel within assigned territory (up to 50%) Valid driver's license, reliable vehicle, and current auto insurance Ability to lift/push/pull up to 50 lbs and assist with patient mobility Associate's or Bachelor's in Nursing preferred Home Health, Hospice, or HomeCare HomeBase experience preferred Why Join Elara Caring?
