In this vital role, you will be responsible for assessing incoming patients, coordinating admission processes, and ensuring seamless transitions from referral or hospital settings into our care facility. Your expertise in acute and post-acute care environments will help facilitate efficient patient intake, accurate documentation, and effective communication with multidisciplinary teams. Responsibilities Conduct comprehensive patient assessments upon admission, including vital signs, medical history, and current condition evaluations. Collaborate with physicians, case managers, and other healthcare professionals to determine appropriate care plans and discharge strategies. Manage patient records accurately, ensuring compliance with HIPAA regulations and hospital policies. Coordinate discharge planning by evaluating post-acute care needs, arranging follow-up services, and communicating with families and external providers. Assist in diagnostic evaluations and medical record documentation to support clinical decision-making. Maintain a high level of patient service by providing compassionate support during the admission process while adhering to hospital protocols. Skills Strong knowledge of medical terminology, physiology, and acute care procedures. Experience in hospital medicine, post-acute care, outpatient clinics, or managed care environments. Proficiency in patient assessment techniques including vital signs monitoring and diagnostic evaluation. Ability to navigate complex healthcare systems involving case management, utilization management, discharge planning, and medical records management.
