Admissions Nurse (RN) | Auburn Crest Hospice Be the First Person a Family Trusts The admission visit is the moment a family stops feeling alone. Done well, it changes everything that follows. We are looking for an experienced RN who can walk into a hard room, explain hospice with clarity and compassion, and give families confidence in the care ahead. What You'll Do Perform hospice consultations and admissions, assessing eligibility and appropriateness of care Educate patients and families on hospice services and patient rights, with patience and honesty Communicate with physicians to secure admission orders, medications, and recertifications Participate in the Interdisciplinary Team Conference and plan of care review every 14 days Support documentation review to ensure patients continue to meet hospice qualifying criteria Provide clinical supervision, competency evaluations, and orientation support for nursing staff and CNAs Contribute to our QAPI program and to in-service education for staff and community partners Carry a part-time case load as assigned What We're Looking For Active RN license in Idaho Two or more years of nursing experience, with hospice or home health experience strongly preferred. If you have worked in hospice, home health, palliative care, oncology, or long-term care, we want to hear from you.
