Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage members Review, evaluate, and document medical history, current conditions, and medication usage Educate patients regarding their diagnoses, health status, and prescribed medications Identify and communicate gaps in care, offering recommendations for optimal health outcomes No requirement to prescribe medications or develop ongoing treatment plans

Qualifications:

Active Nurse Practitioner license in Idaho Board Certification through AANP or

ANCC MSN

degree from an accredited nursing program Current CPR certification (BLS) Minimum of 6 months of current NP experience preferred Compensation for this per diem role is structured at $80$90 per evaluation, with additional mileage stipends and administrative time included to recognize your commitment and travel.
