Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage members Review and evaluate medical history, chronic and acute conditions, and medication use Deliver education to members about health conditions and appropriate medication usage Identify care gaps and recommend actions to address them No treatment planning or medication prescribing required

Qualifications:
