Key Responsibilities:

Perform comprehensive annual health risk assessments in patients homes Assess and document prior and current medical history, existing conditions, and medication use Educate patients about their health conditions and medications Identify and report any gaps in care, providing actionable recommendations Focus exclusively on assessmentno treatment planning or prescribing required

Qualifications:

Active Nurse Practitioner license in Idaho Valid CPR/BLS certification Board Certification via AANP or

ANCC MSN
