Flexible shifts available Monday through Sunday Choose and build your own schedule Minimum commitment on a weekly or bi-weekly basis

Key Responsibilities:

Conduct annual home health risk assessments in patient homeseach visit approximately 45 minutes Obtain and review medical history, current medical conditions, and medication information Provide education and counseling on chronic conditions and medication adherence Identify gaps in care and recommend next steps to enhance wellness and safety No requirement to create care plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in the state of Idaho Current CPR certification (BLS) Board certified through AANP or

ANCC MSN

degree from an accredited nursing program At least 6 months of current Nurse Practitioner experience preferred This role offers competitive compensation at $80$90 per completed evaluation, with additional mileage reimbursement and paid administrative time.
