Navitas Healthcare, LLC is seeking Registered Nurse (RN)
  • Home Health for an exciting Travel or Local job in Boise, ID.
Key Responsibilities Provide skilled nursing care to patients in their homes in accordance with physician orders and individualized care plans Conduct comprehensive patient assessments and ongoing evaluations Develop, implement, and update patient care plans based on clinical findings and patient needs Complete accurate OASIS documentation and maintain timely electronic medical records Administer medications, wound care, treatments, and other nursing interventions as prescribed Educate patients and family members regarding medications, disease management, treatment plans, and home care instructions Coordinate care with physicians, therapists, case managers, and interdisciplinary healthcare teams Monitor patient progress and report changes in condition to appropriate providers Ensure compliance with home health regulations, infection control standards, and agency policies Maintain productivity standards and manage daily scheduling efficiently Required Qualifications Active Registered Nurse (RN) license in the applicable state Current BLS/CPR certification required Minimum of 1 year of recent Home Health RN experience required OASIS documentation and billing experience required Experience with Homecare Homebase software required Strong clinical assessment and care coordination skills Excellent communication, time management, and documentation abilities Ability to work independently and manage a patient caseload effectively Reliable transportation and valid driver's license may be required for patient visits For more details contact at or Call / Text at 516-862-1169.
