Job Description

Registered Nurse (RN) Hospice Case Manager Oakstone Hospice is seeking a compassionate and skilled RN Hospice Case Manager to join our interdisciplinary team. This role provides direct patient care, care coordination, and support to patients and families throughout the end-of-life journey. Responsibilities Manage a caseload of hospice patients in home and facility settings Complete initial and ongoing nursing assessments Develop and update individualized plans of care Provide symptom and pain management focused on comfort and quality of life Coordinate care with physicians, social workers, chaplains, CNAs, and other team members Educate patients and families on medications, disease progression, and end-of-life expectations Respond to changes in patient condition and provide timely interventions Complete accurate, timely documentation per hospice and Medicare regulations Participate in interdisciplinary team meetings Qualifications Active RN license in the state of practice Minimum 1-2 years of nursing experience Hospice experience preferred minimum of 1 year (training available for the right candidate) Strong assessment, communication, and time-management skills Ability to work independently in the field Valid driver's license and reliable transportation What We Offer Competitive pay Supportive leadership and manageable caseloads Ongoing training and education Meaningful, patient-centered work in a team-focused environment Join Oakstone Hospice and make a difference where it matters most.
Pay:
$80,000.00 - $90,000.00 per year
