Key Responsibilities:

Conduct annual home health risk assessments for Medicare Advantage plan members in various home settings Assess patients prior and current medical histories, underlying conditions, and medication use Deliver clear patient education regarding diagnoses and medication management Identify care gaps and offer recommendations, promoting improved overall health Focus solely on assessmentsno need to create ongoing treatment plans or prescribe medication

Qualifications:

Valid Nurse Practitioner license in Idaho Current BLS/CPR certification Board certified via AANP or

ANCC MSN
