An excellent opportunity is available for an experienced Nurse Practitioner to provide home health services on a per diem basis throughout Cassia County. In this role, you will conduct comprehensive home health risk assessments for Medicare Advantage plan members residing in various locations, with schedule flexibility tailored to your needs.

Schedule:

Flexible shifts available Monday through Sunday Tailor your own work schedule Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

Perform home health risk assessments, each averaging 45 minutes Travel between member homes in Burley, Oakley, Declo, and other areas within Cassia County Assess members with a focus on annual health evaluations or post-discharge follow-up Review prior and current medical history, ongoing conditions, and medication use Educate patients on medical conditions and medications to ensure understanding and compliance Identify care gaps and suggest recommendations for improvement No need to develop treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Idaho Current CPR certification (BLS) Board certified by AANP or ANCC Minimum 6 months experience as a Nurse Practitioner preferred MSN degree from an accredited nursing program In return for your expertise, you will receive compensation of $80-$90 per evaluation, along with a mileage stipend and administrative time to ensure your efforts are recognized and rewarded.
