Key Responsibilities:

Perform comprehensive annual risk assessments for Medicare Advantage enrollees in their homes Review and evaluate prior and current medical history, existing health conditions, and medication regimens Educate patients on their health conditions and prescribed treatments Assess for gaps in care and recommend appropriate next steps No ongoing treatment plan development or medication prescribing is required

Qualifications:

Active Nurse Practitioner license in Idaho Current BLS/CPR certification Board certification through AANP or ANCC At least 6 months of recent NP experience (preferred) MSN degree from an accredited nursing program This position compensates $80-$90 per evaluation, plus mileage reimbursement and payment for administrative time.
