Key Responsibilities:

Conduct comprehensive annual home health risk assessments during in-home visits for Medicare Advantage plan members Review and document prior and current medical history, existing conditions, and medication usage Deliver patient education on diagnosed health conditions and medication regimens Spot potential gaps in care, offering clear recommendations to improve patient outcomes No requirement for developing treatment plans or prescribing medications

Qualifications:

Valid Nurse Practitioner license in Illinois Board Certification (AANP or ANCC) Current CPR certification (BLS) MSN degree from an accredited nursing program At least 6 months of recent Nurse Practitioner experience is preferred This position rewards your expertise at $80$90 per evaluation, plus a mileage stipend and compensation for administrative time.
