Delivering top-tier home health care is central to this per diem Nurse Practitioner position focused on supporting Medicare Advantage plan members throughout Madison County, Illinois. Youll be responsible for conducting comprehensive risk assessments in patients homesincluding areas such as Granite City, Edwardsville, and Alton. These visits, typically lasting about 45 minutes, may be scheduled as annual health assessments or post-discharge evaluations.

Key Responsibilities:

Conduct annual and post-discharge home health risk assessments for Medicare Advantage members Review patients prior and current medical history, chronic conditions, and medication lists Educate patients regarding health status, disease management, and medication adherence Identify care gaps and make recommendations to improve patient outcomes No requirement for treatment planning or prescribing medications

Qualifications:

Unencumbered Nurse Practitioner license in Illinois Current CPR certification (BLS) Board certification via AANP or

ANCC MSN
