Role Location:
Transitional Care Clinic Memorial Hospital Belleville/Shiloh 4500 Memorial Drive, Suite C 100 Belleville, Illinois 62226 The Transitional Care Clinic (TCC) program focus is on chronic disease management and readmission prevention for high-risk patients with heart failure (HF), chronic obstructive lung disease (COPD), and end stage renal disease (ESRD). TCC staff meet with inpatients at the bedside to provide chronic disease education, management, and care coordination for discharge and outpatient follow up. TCC staff work closely with multidisciplinary staff both inpatient and outpatient for optimal health management. Post discharge contact is provided through telephonic, video, and clinic follow-up. TCC staff evaluates and solve barriers around SDOHs identified. TCC Team includes RNs, NP, Pharmacist, CHW, SW, and Dietician.

It provides patients a full complement of diagnostic and treatment services as well as heart and vascular care. Memorial, offering medical and surgical services plus critical care, is an accredited Chest Pain Center with PCI by the Society of Cardiovascular Patient Care and is designated as an Acute Stroke Ready Hospital by the Illinois Department of Public Health. In addition, Memorial Belleville recently was accredited by the American College of Radiology as a designated Lung Cancer Screening Center. Since 2008, it has been designated as a Magnet
  • recognized organization for nursing excellence by the American Nurses Credentialing Center.
Memorial Hospital Shiloh , a 94-bed, all-private suite hospital was recognized with Magnet status in 2018 and provides emergency care, labor & delivery, nursery, medical and surgical services plus critical care. Care Management includes Utilization Management (Utilization Review), Discharge Planning, Social Services, Clinical Outcomes (Care Managers), Discharge Transition Coaches, Nurse Practitioners, and Clinical Documentation Improvement. Preferred Qualifications Role Purpose Coordinates, negotiates, procures and manages the care of patients to facilitate achievement of quality cost-effective outcomes. Works collaboratively with physicians and interdisciplinary staff, internal and external to the organization, to identify and resolve issues. Performs and provides consultation regarding Utilization Management and Performance Improvement programs in order to obtain optimum value for the patient and the reimbursement source. Responsibilities Assesses, coordinates, and evaluates each patient's plan of care. Establishes, continuously evaluates, and documents an individualized discharge plan. Ensures clinical documentation and cost-effective use of hospital resources.
