Perform initial home health evaluations and ongoing reassessments as needed. Develop and implement individualized plans of care. Complete OASIS documentation accurately and in a timely manner. Supervise Home Health Aides (HHAs) and provide clinical guidance as appropriate. Ensure compliance with Medicare regulations and agency policies. Communicate changes in patient condition to physicians and other healthcare providers. Coordinate care with interdisciplinary team members to support optimal patient outcomes. Complete all required documentation and paperwork efficiently and on schedule. Educate patients and caregivers regarding disease processes, medications, treatments, and care plans. Deliver high-quality nursing care in accordance with professional standards and agency guidelines. Provide skilled nursing services, including wound care, IV therapy, line maintenance, and ongoing assessments. Collaborate effectively with healthcare professionals, patients, and family members to ensure continuity of care. Qualifications Current and unrestricted Illinois Registered Nurse (RN) license. Home health experience preferred. Proficiency with OASIS documentation. Knowledge of Medicare regulations and standards. Excellent clinical assessment, communication, and organizational skills.
