Deliver quality patient care by performing home health risk assessments as a Nurse Practitioner on a per diem basis. This role focuses on providing comprehensive evaluations for Medicare Advantage health plan members in Ogle County. With flexibility at the core, you can build your ideal work schedule conducting assessments in locations such as Rochelle, Byron, and Oregon. Each home visit, lasting about 45 minutes, ranges from annual health reviews to post-discharge assessments, ensuring a diverse and impactful caseload.

Responsibilities:

Complete annual home health risk assessments for Medicare Advantage members in their homes Review and document patients prior and current medical history, conditions, and medication use Educate patients on their health conditions and proper medication management Identify care gaps and make recommendations to improve patient outcomes Focus exclusively on assessmentsno requirement for developing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Illinois Certified in Basic Life Support (BLS/CPR) Board certification through AANP or ANCC Master of Science in Nursing (MSN) from an accredited program At least 6 months of Nurse Practitioner experience preferred
