Your Role:

Coordinate resident assessments, MDS completion, and care planning processes Ensure accurate documentation for reimbursement and regulatory compliance Collaborate with interdisciplinary teams to ensure quality resident care Develop, review, and revise individualized care plans as needed Monitor resident progress and coordinate the continuum of care Support regulatory compliance and maintain current knowledge of long-term care practices Help ensure residents receive the resources and services needed for optimal outcomes

Who We're Looking For:
