Full Job Description Position Overview:
The Hospice Field RN provides compassionate end-of-life care as part of an interdisciplinary team, coordinating and delivering direct and indirect patient care in patients' homes across multiple counties.
Key Responsibilities:
Perform assessments, care planning, monitoring, and evaluation in accordance with regulatory standards. Communicate effectively with patients, families, and healthcare team members, ensuring timely documentation and adherence to policies. Supervise home health aides, assign tasks, and provide guidance in patient homes. Assess patients for hospice placement, manage symptoms, and provide comfort measures. Participate in case conferencing, coordinate care, and facilitate communication among team members. Conduct home visits for patient monitoring, end-of-life assessments, and post-mortem procedures.
Qualifications & Skills:
Licensed Registered Nurse (RN) with Illinois certification. Current Basic Life Support (BLS) certification. Valid driver's license and auto insurance. Minimum of 1 year of nursing experience. Strong understanding of hospice regulations, documentation standards, and patient care planning.
Key Duties:
Deliver holistic end-of-life care, ensure compliance with regulatory requirements, document accurately, coordinate with the healthcare team, and support families through compassionate patient care and bereavement referrals.
