Transitions Care is seeking a compassionate Registered Nurse for Hospice After Hours to provide expert clinical care, triage, and support to patients and families during evenings, nights, and weekends. In this role, you will perform symptom assessment and management, respond to urgent needs, make home or facility visits, and coordinate with the interdisciplinary team to ensure continuity of care. You'll educate and support caregivers, document thoroughly, and uphold our mission of dignity, respect, and comfort at end of life while working autonomously within a collaborative, values-driven organization. Responsibilities Provide after-hours hospice nursing care to patients in homes and facilities Perform triage via phone and in-person to address urgent patient and family needs Conduct comprehensive assessments and manage symptoms, including pain and distress Make home and facility visits during evenings, nights, weekends, and holidays Collaborate with the interdisciplinary hospice team to ensure continuity of care Educate and support patients, families, and caregivers on disease process and care plans Document assessments, interventions, and communications accurately and promptly Participate in on-call rotation and respond to visits within required timeframes Advocate for patient dignity, comfort, and goals of care at end of life Adhere to hospice regulations, clinical protocols, and safety standards Required Skills Hospice nursing Palliative care Triage and critical thinking Pain and symptom management Home health nursing Electronic medical records (EMR) documentation Care coordination Patient and family education End-of-life care planning Infection control and safety
