Job Summary We are seeking a Registered Nurse to join our team of home health professionals. The successful candidate will be responsible for providing quality care to patients in their homes, assessing and monitoring patient conditions, and providing education and support to patients and their families. The ideal candidate for this position will have excellent communication skills, an ability to work independently, and a commitment to providing the highest level of care.

Requirements:

Illinois license to practice as a RN Minimum of 2 years of nursing experience; hospital and nursing home experience preferred Current CPR certification Verification of current auto insurance Ability to work independently Strong sense of initiative and urgency in addressing patient crises Excellent communication skills Flexibility to perform duties

Responsibilities:

Provide direct patient care in the home setting. Assess patient's health status and develop a plan of care to meet their needs. Educate patients and families on health promotion and disease prevention. Monitor patient's progress and adjust care plans as needed. Administer medications and treatments as prescribed by the physician. Collaborate with other healthcare professionals to ensure quality patient care. Maintain accurate records of patient care, condition, progress, and concerns. Territory Southside Chicago Qualifications Valid Registered Nurse (RN) License 2 Years Nursing Experience Proficient in patient care and assessment Experience with catheterization and IV insertion Knowledge of EHR systems Skilled in medication administration and care plan development Familiarity with home health and community care practices Basic life support certification Experience in long-term care and assisted living settings
